Easement - Separate document or in a deed?

I understand a recorded easement as a standalone document.
But, what happens if an easement is recorded in the body of a Deed attendant
to a sale of the land? Something like an ingress/egress easement over the
land.
Then when the land is sold again and the NEW deed says something like:
"Subject to encumbrances, conditions, easements, ... etc."
but without specific reference to the initial easement on the previous deed?
Does this cloud the initial easement or not? Does it convey by virtue of
the "footnote" and the earlier recorded deed or likely not?
